Add Halo-TeaCache: AMD Unified Memory TeaCache for LTX2

Single-file TeaCache implementation for LTX2 (LTXAV) audio-video generation
on AMD APUs with unified memory. Caches 48-layer dual-stream transformer
output with ~35% speedup on cached steps. No CPU offload needed.
